Community health services at the village level, particularly at Posyandu, are still largely managed manually using register books. This condition often creates several challenges, such as delays in reporting, the risk of data loss, and difficulties in tracking the health records of pregnant women and children under five. Based on these problems, this study aims to design and develop a web-based Posyandu information system using the Waterfall method, enabling the recording and reporting processes to be carried out more effectively, efficiently, and in an integrated manner. The Waterfall method was selected because it provides systematic and structured development stages, including requirements analysis, system design, implementation, and testing using the Black Box Testing method. This research was conducted at three Posyandu locations in Buay Pemuka Peliung District, namely Negeri Agung Village, Kajarang Moncol Village, and Way Handak Village. The results show that the developed system is capable of managing data on pregnant women, children under five, immunization, vitamin administration, and automatically generating monthly reports. Based on the functional testing results, all system features operated properly according to user requirements. Therefore, the developed information system can assist Posyandu cadres and healthcare workers in accelerating data processing and reporting, while improving the efficiency and accuracy of community health services at the Posyandu level.
