Pendidikan merupakan sektor penting yang terus ditingkatkan kualitasnya melalui pemanfaatan teknologi informasi. Di SD Negeri 004 Sungai Manau, khususnya pada kelas IV, V, dan VI, proses ujian masih dilakukan secara manual dengan mendistribusikan soal dan mengoreksi jawaban siswa secara langsung. Proses ini menimbulkan berbagai permasalahan seperti membutuhkan waktu yang lama, biaya tambahan, serta berisiko menimbulkan kesalahan dalam koreksi maupun rekap nilai. Penelitian ini bertujuan untuk merancang dan membangun sistem informasi ujian online berbasis komputer yang dapat mempermudah proses evaluasi pembelajaran. Metode penelitian yang digunakan meliputi analisis sistem berjalan, perancangan sistem dengan use case diagram, activity diagram, sequence diagram, serta perancangan antarmuka. Hasil dari penelitian ini adalah prototipe sistem ujian online yang memungkinkan guru membuat soal, mendistribusikan ujian, serta memperoleh hasil koreksi dan rekap nilai secara otomatis. Siswa dapat mengikuti ujian secara online dan langsung memperoleh umpan balik hasil belajar. Dengan adanya sistem ini, efektivitas, efisiensi, akurasi, dan transparansi proses evaluasi dapat meningkat, sekaligus mendukung mutu pendidikan di SD Negeri 004 Sungai Manau.
